A Self-Assessment Questionnaire
Two forms will need to be completed before your assessment. One form is completed by you, and the second is completed by a family member or someone close to you who has known you for a number of years.
These questionnaires help your ADHD specialist better understand you and your symptoms – for a more accurate ADHD diagnosis and treatment plan.
The ADHD Assessment
The assessment is a 90-minute online video ADHD assessment with your personal Clinician. You can consider it a relaxed, non-judgmental discussion with your clinician about symptoms, medical history, and complaints. The assessment will also include counselling on the various treatment options used to manage ADHD.
Your Assessment Report
The report will detail:
- Diagnostic outcome – this will tell you if you have been diagnosed with ADHD and what type of ADHD it is.
- An explanation of the diagnosis findings.
- Proposed treatment plan and options.
- Information Pack on how to move forward with your treatment plan.

Along with this then also provide you the information to book your first medication/titration appointment. Currently I am still going through titration, my clinician, Annisah, is very thoughtful and I consistently feel like I have the support of an expert to hand while also being able to easily advocate for myself. In my last appointment I felt that a medication wasn't working for me and wanted to switch and Annisah informed me of the alternatives, what I could expect, and what possibilities there were - I asked a crazy amount of questions and she answered them all clearly and without getting frustrated at my indecisiveness. In the end we switched to another medication and I feel very happy about it and I am confident that my ADHD is becoming more manageable. In terms of medication, ADHDCertify has a exclusive contract with a pharmacy. This means that your clinician sends your script to them directly, and then the pharmacy posts it to you. They say 2-5 days but my experience so far has been 1-3 days - the pharmacy seems to use something like Royal Mail 24 Tracked to ensure it comes fast. Overall my experience with ADHD Certify has been clear and good throughout.
