A Self-Assessment Questionnaire
Two forms will need to be completed before your assessment. One form is completed by you, and the second is completed by a family member or someone close to you who has known you for a number of years.
These questionnaires help your ADHD specialist better understand you and your symptoms – for a more accurate ADHD diagnosis and treatment plan.
The ADHD Assessment
The assessment is a 90-minute online video ADHD assessment with your personal Clinician. You can consider it a relaxed, non-judgmental discussion with your clinician about symptoms, medical history, and complaints. The assessment will also include counselling on the various treatment options used to manage ADHD.
Your Assessment Report
The report will detail:
- Diagnostic outcome – this will tell you if you have been diagnosed with ADHD and what type of ADHD it is.
- An explanation of the diagnosis findings.
- Proposed treatment plan and options.
- Information Pack on how to move forward with your treatment plan.

My prescriber is Annisah & she's wonderful, helpful & knowledgeable. She's really helpful with getting to the bottom of things to help improve your symptoms. I don't feel I need to mask in appointments, and she's patient with me waffling on about every detail possible, lol. On top of this the admin are really fast to respond (sometimes I receive responses within the same hour) and accommodating. I wanted to split payments, like paying for appointments & medication separately, & they were happy for me to do this straight away. Some aspects feel pretty expensive, but it is everywhere, and you can try to transfer bits over to the NHS in stages if you are expecting to take multiple medications. If you're looking for a sign to take the plunge with the long waiting times on the NHS, here is a really good option. My assessment with Lisa was also extremely thorough and she was lovely. I was screened beforehand & during for any other mental illness explanations other than ADHD. This level of thoroughness was really what I was hoping for! My biggest tip is - It's best to make a detailed list of issues you've noticed (whether you're sure it's ADHD or not) to bring to your assessment. You can send it over to them beforehand too. If you have had any record of any sort of birth complications however minor, they'll ask you about this too so bring this!
